I hereby direct that the distribution(s) described above be made from the Coverdell ESA(s) referenced above. I understand and acknowledge that Victory Funds (including the Custodian of my Coverdell ESA) will be acting solely on my representations and instructions as set forth in this form, and that Victory Funds (including the Custodian of my Coverdell ESA) will not independently verify whether or not I qualify for the type or amount of distribution directed. I understand and acknowledge that I alone am responsible for all tax consequences that may result from taking such distribution(s) and that I should seek the advice of an attorney or tax advisor regarding my specific situation.

If I have selected EFT of my distribution by Victory Funds, I authorize Victory Funds to begin, and the financial institution named to accept, electronic deposits (credits) and withdrawals (debits) to my designated account(s) and to reverse, if necessary, any deposits or withdrawals made in error to my account(s). Automated debit or credit entries shall constitute my receipt for the transaction(s). This authority is to remain in full force and effect until Victory Funds has received written or phone notification from me of its termination at such time and in such manner as to give Victory Funds reasonable opportunity to act on it. I also understand Victory Funds may terminate the EFT instructions from my mutual fund account(s) at any time in its sole discretion. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.

For your protection, we may require additional verification of your identity and/or instructions before accepting your transaction. The amount of money that you receive will depend on the execution price once we have verified your identity and/or instructions and the order is in proper form. Due to market fluctuations while we obtain verification, the amount you ultimately receive could be less than the amount at the time you initiated this transaction. If we are unable to reach you to verify this transaction within five days, we may cancel this transaction. We are not liable for any delayed executions, canceled orders, or requests not acted upon as a result of the verification of your identity and/or instructions.

Important: Total distributions/transfers will be subject to a $20 fee. Total distributions due to death or disability are not subject to this fee.
